H&S Surveys (general)

These surveys below are NOT the official reports you need to complete rather a mechanism for you to let us know that you are completing these reports. If you are completing these forms we need to know!

Classroom Evacuations

In some work-sites especially we are hearing that classroom evacuations are all too frequent of an occurrence. There are no mechanisms, however, to track these situations so we’ve created a reporting form which will take you less than two minutes to complete. We think it is dysfunctional for classroom evacuations to be happening – certainly on a regular basis and want to assist you. We need you to complete this so we can.

Safe Schools Form

This form is an internal form to the school and again, there is no system data of the usage and effectiveness of these forms. Completing this survey will take less than 1 minute. Generally, we want to know: 1) how often they are being used, 2) are you being responded back to by your administration (as they are required to by the Ministry of Education PPM)

Violent incident Reports

The forms that you complete online are sent to the UCDSB Health and Safety Officer and principal as you submit it online. Currently we only receive summary information monthly and we need to have more timely information. Your completion of this form will assist us to intervene much more quickly.

Indoor Air Quality (IAQ)

Maintaining an excellent air quality is vital to everyone’s health. Poor IAQ can result in headaches, fatigue, irritability, nausea, dizziness, diminished ability to concentrate and can contribute to specific  problems related to asthma, sinusitis, and other respiratory diseases. If you have any concerns with the air quality in your worksite please review the Board’s policy and complete ETFO-UCL’s survey.
